Download, sign and email the Volunteer Agreement to rsvp@onestone.org by Thursday, October 12.

Step 3:
If you would like your student to receive any necessary immunizations as determined by the Central District Health Department, please download the Medical Release Form and email it to AMunoz@cdhd.idaho.gov by Thursday, October 12.
Alternatively, you may mail the form to: Central District Health Department, ATT: Alisha Munoz, RN, 707 N. Armstrong Pl, Boise, ID 83704

As part of this event, all students will have the opportunity to get up-to-date on any necessary immunizations based on the IRIS (Idaho’s Immunization Reminder System). Immunizations will be professionally administered. There is no cost for the vaccine. The administration fee will be billed to insurance and any remaining charged will be covered by the Idaho Immunization Program.
